USF Students with Disabilities Currently Receiving Services

Ms. Burnam's Fall 2008 Welcome Back Student Letter.

We are here for you! To get the most of your accommodations, please follow the USF Student Handbook and the procedures/policies of Students with Disabilities Services (SDS). This website is meant to clarify the procedures and policies of SDS. Please contact the office if you have any questions or problems.


Student Responsibilities For Each Semester

Schedule as appointment with a SDS coordinator within the first week of the new term

The student is encouraged to schedule an appointment within the first week of the semester to review his/her courses and determine the accommodations necessary in each course.

Request a SDS Memo of Accommodations (SDS Memo) for each class

The student will complete an Accommodation Request Form listing each class he/she is registered. SDS will prepare an SDS Memo for each of the student’s professors.

Deliver the SDS Memo

The student is expected to deliver the SDS Memo to each of his/her professors. SDS recommends that the student request an appointment during the professor's posted office hours and present his/her SDS Memo in a confidential setting.

Use his/her accommodations

Throughout the term, the student is expected to consistently use the accommodations agreed upon between the student, SDS, and the professor. Remember the time frames necessary to arrange the accommodations and keep on schedule.

Schedule your Accommodated Testing in advance

The students receiving Accommodated Testing through SDS are expected to meet with each professor at least two (2) weeks before the date of the first examination to discuss his/her exam accommodations and together complete a Faculty Exam Request Form (FERF). Students are expected to submit each completed FERF to SDS at least five (5) weekdays before the first examination. Faculty members are expected to deliver exam materials to SDS at least two (2) hours before the examination. Students are expected to submit any schedule changes in writing on an Exam Rescheduling Form.

Self-Monitor

The student is encouraged to self monitor his/her progress in the classroom. If the accommodations are not effective, the student should consult with a SDS Coordinator to determine if adjustments or additional techniques can be more effective.

Student Instructions for each Semester

At the start of each semester

  1. Turn in your Request for Accommodations Form to Students with Disabilities Services (SDS);
  2. Pick up your SDS Memo of Accommodations (SDS Memo) for each professor and the Faculty Receipt Form from SDS;
  3. Pick up a Faculty Exam Request Form (FERF) for each classfrom SDS;

Next

  1. Deliver your SDS Memos to your professors within two weeks of receiving them from SDS;
  2. Make sure that each professor signs the Faculty Receipt Form;
  3. Return the Faculty Receipt Form with signatures from all your professors to SDS;

To take your exams in SDS

  1. Meet with each professor at least two (2) weeks before your first exam;
  2. Discuss your exam accommodations with your professor and together complete a FERF;
  3. Give your professor the last sheet of the FERF after completion;

Then

  1. Deliver the completed FERF to SDS at least one (1) week prior to the exam;
  2. Keep your copy of the FERF ;
  3. Contact your instructor and SDS at least two (2) days prior to your examination to confirm arrangements;

On day of your exam

  1. Arrive at SDS 15 minutes before your scheduled examination time;
  2. Bring only what you need for the exam;
  3. After the exam, provide feedback to the SDS staff about the exam accommodations;

Making changes to your exam schedule

Any change in exam times or dates must be indicated on an Exam Rescheduling Form and authorized by the instructor. The form must be submitted to SDS in a timely manner.

Appeal Procedure

If you want to appeal decisions about services or accommodations provided by Students with Disabilities Services (SDS), there is a process to resolve your issues under the University's ADA Policy (See USF ADA POLICY 0-0008A at usfweb.usf.edu/usfgc/gc_pp/genadm/gc008a.htm). A copy of this policy is available on the USF web site and in SDS. Please talk with one of the coordinators at SDS as soon as possible in order to process appeals quickly and fairly.
